Effective event investigations are fundamental to patient safety, organizational learning, and risk reduction. While many organizations emphasize investigative frameworks such as Root Cause Analysis, the quality of an investigation ultimately depends on the quality of the information collected.
This presentation explores best practices for conducting comprehensive healthcare event investigations, with a particular focus on cognitive interviewing as an evidence-informed method for gathering accurate, unbiased, and complete information from patients, staff, and witnesses.
Participants will examine how investigative planning, interview preparation, evidence collection, documentation, analysis, and cognitive interviewing work together to produce more reliable findings, stronger identification of contributing factors, and more effective corrective actions.
Through practical examples and real-world applications, attendees will learn strategies to improve the consistency, credibility, and effectiveness of healthcare event investigations.

With more than 25 years of healthcare experience, including 18 years in risk management and patient safety, she has served in executive leadership roles, including Chief Risk Officer and Patient Safety Officer. An emergency and trauma nurse by background, Myka is a nationally recognized speaker and educator who serves on the ASHRM Advisory Board and National Teaching Faculty and as a Board Director for the Florida Society for Health Care Risk Management and Patient Safety.
