This session will explore how payment models and health system acquisitions of rehabilitation or senior care facilities and outpatient services such as home health agencies impact financial outcomes, care transitions, patient safety, quality, consumer satisfaction, and risk management.
This session will look at the impact that value-based care, bundled payments, and pay for performance models have on an organization's operating margins, risk, and patient safety. Specifically, it will explore risks related to hospitals' acquisitions of rehabilitation, long term care and/or home health/hospice companies, including the risk of re-admissions due to systemic care transition failures. Risk managers and patient safety officers can have a positive financial impact on the health system by using specific strategies for ensuring successful transitions in care and promoting quality and safety throughout the care continuum.
