Health Care Reform and Its Impact on Transition of Care

This session will look at the impact that value based care, bundled payments and pay for performance models have on an organization's value, risk and patient safety. Specifically, it will explore risks related to hospitals' acquisitions of rehabilitation, long term care and/or home health/hospice companies, including the risk of re-admissions due to systemic care transition failures. Risk managers and patient safety officers can build value for organizations by using specific strategies for ensuring successful transitions in care and promoting quality and safety throughout the care continuum.

  • Identify and address risks associated with higher acuity patients in residential and out patient care settings

Bio

A registered nurse, Martinez holds multiple licenses and certifications in nursing and risk management and is an active member of the American Society for Healthcare Risk Management (ASHRM) as well as a frequent speaker at industry trade conferences. For ASHRM, she’s served on the Conference Planning Committee, the Ethics Committee, the 2030 Fund, the Chapters Leaders Task Force and as Chair of the New Member Task Force.

Bio

Michelle is from Austin, Texas and is the CEO of OmniSure Consulting Group, a firm that for 19 years has been contracted by medical professional insurance carriers and risk retention groups to provide risk management services for community hospitals, medical facilities, senior care facilities, allied and miscellaneous healthcare professionals. Before launching OmniSure in 2000, Michelle progressively held multiple positions in Healthcare and Senior Living where cleared up regulatory issues in high risk facilities, improved quality, redesigned models of care, and repaired the reputation of a number of facilities. She received her BA in Psychology from the University of Houston, did her post graduate certification in nursing facility administration with the University of North Carolina at Chapel Hill. She holds an Associate in Risk Management and is a licensed property casualty agent. She has investigated sexual abuse, written articles on the topic, and her company does sexual abuse liability risk assessments.
