The manual contains policies on disclosure of adverse event, non-punitive reporting, patient safety tracking tool (PSTT) and failure mode/effect/criticality form, , and assists in meeting JCAHO patient safety standards. It has been well received and is available to anyone, providing credit is given to PAHCRM. It can be obtained from jbortner@haponline.org.
PAHCRM is also working with the Hospital and Healthcare Association of Pennsylvania to review draft legislation on peer review and patient safety. Four affiliated societies have held sessions on patient safety, OSHA, and new developments in advance directives. Washington
The 2001-2002 Washington Health Care Risk Management Society (WHCRMS) board of directors has been working on several issues over the past few months. WHCRMS has heard from members who are concerned about the new JCAHO accreditation standards that require disclosure of unanticipated outcomes. The board provided an educational opportunity in June to learn more about the issue of disclosing unanticipated outcomes to patients by hosting the ASHRM satellite conference on this issue. Also, to kick off our 2001 fall monthly membership meetings, September 21 meeting at Highline Hospital will also address disclosure of unanticipated outcomes.
Finally, this chapter is pleased to be the host chapter for the 2002 ASHRM annual conference in Seattle. Be sure to visit the WHCRMS booth at the conference in Boston this year to learn more about September 29 - October 1, Seattle in 2002!

James L. Groves
Jim Groves, the American Hospital Association's (AHA) first risk manager, died after a long illness on August 6, 2001, in Scottsdale, AZ. He was 69.
Jim was a pioneer who promoted hospital-formed insuring organizations (HFIOs) to provide hospitals with self-insurance alternatives. This was in response to the massive hospital and medical professional liability insurance market shrinkage that produced the great malpractice crisis of 1975. Jim realized that the HFIOs (which included both captive insurance companies and self-insured trust funds) would be successful only if hospitals took steps to prevent losses on the front end through effective loss prevention programs.

ASHRM's Advocacy Activities Continue
|
In August, ASHRM members received a member alert focusing on "an immediate and growing workforce shortage." Referring to the growing shortage of nurses, pharmacists, and radiological technologists, the communication solicits some 17,000 members of the American Hospital Association's (AHA) Personal Membership Groups (PMGs) to send postcards to the White House and Capitol Hill seeking action on several pending healthcare workforce shortage-related bills. The Hospital Caregivers Postcard Campaign was launched by the AHA, Association of American Medical Colleges, Catholic Health Association of the United States, Federation of America's Hospitals, National Association of Public Hospitals, Premier and VHA, Inc.
Packets were sent to chief executive officers (CEOs), which included Help Wanted postcards for delivery to U.S. Senators, Representatives and President Bush. ASHRM members were encouraged to work with their CEOs to sign and mail the postcards in August. For more information about the 2001 Hospital Caregivers Campaign, go to www.caregivershortage.org/PostIndex.asp.
In August, ASHRM members received information on the American Hospital Association Political Action Committee (AHA-PAC). We have included the information and reply envelope with this newsletter because AHA-PAC is a critical component of ASHRM's advocacy program. The second largest healthcare PAC in the nation, AHA-PAC raises and spends nearly two million dollars per election cycle. Contributing to the AHA-PAC is the best vehicle that ASHRM members and other healthcare professionals have to ensure a political voice and presence in Washington. To read more on the AHA-PAC, go to www.aha.org.

CPHRM RENEWAL
Now that over two hundred and fifty individuals have successfully earned the Certified Professional in Healthcare Risk Management (CPHRM) credential, it is time to think about how to maintain the certification. The renewal cycle for the Certified Professional in Healthcare Risk Management (CPHRM) credential is three years, with expiration on the last day of the month in which certification expires.
Renewal may be achieved either through participation in acceptable professional education (Alternative I) or through retaking and passing the CPHRM Examination (Alternative II). A certificant who fails to renew certification is no longer considered certified and must relinquish his/her certificate and certification pin and cease using the certification credential and merchandise representative of having achieved certification. A certificant who fails to renew before expiration may regain certification only through re-examination.
Alternative I - Participation in Continuing Professional Education:
To renew through continuing professional education, a certificant must:
- Demonstrate participation in 45 contact hours of continuing professional education in categories identified in the CPHRM Examination content outline since initial certification or most recent renewal;
- Attest to adherence to Professional Standards of Conduct; and
- Pay the renewal fee of $85 for a member of an AHA Personal Membership Group or $120 for a non-member.
To be accepted for renewal of the CPHRM, continuing professional education must directly relate to the categories identified in the CPHRM Examination content outline. The content outline and tasks that relate to each category are identified in the CPHRM Candidate Handbook (available from www.ashrm.org). ASHRM offers a multitude of acceptable continuing education activities throughout the year that meet the requirements towards CPHRM renewal.
Alternative II - Successful Re-Examination:
To renew through re-examination, a certificant must:
- Pass the Certified Professional in Healthcare Risk Management Examination within one year prior to expiration of certification (subject to usual fees and provisions for testing); and
- Attest to adherence to Professional Standards of Conduct.
For further information about the CPHRM certification or renewal, please contact Maribeth Casey, Executive Director, AHA-CC, by phone at (312) 422-3715, or via e-mail at mcasey@aha.org.
New CPHRM Study Guide Available Soon!
The second edition of the CPHRM Study Guide will be launched the Annual Conference and Exhibition, Boston, MA, October 29 - November 1, 2001. The guide will also be available at www.ahaonlinestore.com, or by calling 1 (888) 242-2626.
This study guide is adapted from the third edition of ASHRM's Risk Management Handbook for Health Care Organizations. The guide has been enhanced to include two practice tests that correspond with the CPHRM exam content outline, as described in the CPHRM Candidate Handbook. The practice tests are referenced to the second and third editions of the handbook.
CPHRM Study Session in Boston
ASHRM will offer a study session designed to assist risk managers in preparing to take the CPHRM exam. The session will be conducted by Bob Bunting and Joyce Benton, lead authors of the CPHRM study guide.
The session is open to all members, at no additional charge. Just indicate participation on the annual conference registration form. The session will begin with a brief overview of the exam, followed by some simple tips for preparing for and taking the exam. The majority of the session will use a familiar game show format to determine "Who Wants to be a CPHRM?" Volunteers from the audience will be asked to serve as contestants to answer sample questions. This interactive format is designed to be informative as well as fun.
Join your colleagues in Boston on the first day of the conference, Monday, October 29. The CPHRM study session will be offered at 9:45 AM, and you will be finished in time for the Chapter Networking Lunch.
Grecelda Buchanan, Administrative Assistant
Joy Roney, Administrative Assistant
When approached about the possibility of featuring a biographical profile in Forum, Grecelda Buchanan and Joy Roney, ASHRM's unflappable administrative team, were undaunted. In fact, sticking to their guns, Grecelda and Joy promptly replied that as a team, any such profile should include not one, but both halves of this dynamic duo.
There is no doubt that the consistent contribution of these two individuals to the society serves not only as support, but as the backbone of ASHRM operations. Between them, Grecelda and Joy have more than fifteen years of experience with the society. Grecelda has been with the American Hospital Association (AHA) for twelve years, eleven of those working with ASHRM. Joy Roney joined the team over four years ago, on a part time basis.
Grecelda is the ASHRM staff historian. "I have seen a lot of changes at both AHA and ASHRM," she explains. "I've worked with four ASHRM executive directors: Trudy Goldman, Christy Kessler, David Strickland, and now Liz (Elizabeth Summy)." Grecelda was born and raised in Chicago and has no plans to leave. When asked about the ups and downs of her position at ASHRM, she notes "I think the best part is the friendships formed with both staff and members. I love hearing from members whom I haven't spoken to for a few years."
She is the proud parent of two children, Briana,10, and Jeremy, 7. She is determined to guide them through life with an "iron fist" of love, education and perfect attendance records at school, she explains. And at 4'10", Grecelda will most likely have to look up to Briana and Jeremy. As all of her colleagues know, Grecelda loves to read, chew bubble gum, and, most of all, eat microwave popcorn and drink Pepsi. Without hesitation, she is the single staff member that all board members, past and present, rely on most. She is always available to assist them in making travel arrangements, and the endless scheduling of conference calls. She works closely with printing services on the coordination of all the ASHRM mailings, and, as she says herself, "if you want it done, go to Grecelda."
Joy came to ASHRM with 30 years experience in architecture, advertising and interior design, as office manager, bookkeeper, traffic manager and expeditor. She has had a lot of client contact and provides top-notch customer service to the many members she speaks with each day. As Joy notes "I am more than willing to 'troubleshoot' until the problem is solved." In addition, she coordinates advertising for the Journal of Healthcare Risk Management and Forum.
Joy has a great love for animals. She is an avid reader and enjoys the outdoors. Joy and her husband are very proud of their summer cottage in South Haven, MI, where they spend every weekend from May through October.
A vast majority of ASHRM members have chatted with Grecelda or Joy at some point, and they would agree with the two when they say, "we do everything possible to keep ASHRM running with guidance and a smile."

MICHELLE HOPPES, RN, MS, AHRMQR
Director of Risk Management,
Michigan Hospital Association Insurance Company
Michelle, how did you enter the field of healthcare risk management?
I entered the field of Risk Management as a patient representative. In this position, I began to learn the fine balance between patient advocacy and risk management. I have always viewed my primary role in healthcare as one of a patient advocate - first in my clinical role as a nurse and now as a risk management professional.
What positions have you held prior to entering risk management?
I have worked in the healthcare field my entire life. As a teenager, I started out as a hospital switchboard operator and then transferred to admitting-registration. I also attended a career center in high school and completed my first year of nursing school. I graduated as an L.P.N. shortly after high school and began to work in pediatrics. I completed my Registered Nurse degree and worked in the areas of critical care, cardiology, surgery and obstetrics.
Describe your education and training.
My clinical education and training was an excellent foundation for risk management. It encouraged me to focus my priorities towards clinical risk reduction to improve patient safety and to reduce financial loss at the same time. I obtained certifications in risk management, quality improvement and recipient rights. I then completed my graduate degree from the Chicago Medical School in healthcare management with an emphasis in healthcare risk management.
Who were your mentors in healthcare risk management? How did they influence your risk management style?
My mentors in risk management were many - my first boss who taught me "everything you ever wanted to know" about the foundation of risk management; my insurance risk and claims consultants who taught me the "ropes" and showed me the benefits of proactive risk management; another mentor was a supervisor later in my career who taught me professionalism, the skill of negotiation, the utmost importance of relationships and the ability to effectively communicate. The most important mentor I have had is my mother - she influenced my style and my character. She is an excellent role model - with an extensive healthcare background, she was a director of nursing for over 25 years.
What involvement have you had in professional organizations?
I have been involved in many professional organizations, starting with the Michigan Patient Representative Society, the Recipients Rights Association of Michigan and the Michigan Society of Healthcare Risk Managers. I have held many positions in MSHRM - past president and most recently as the chair of council of past presidents. I am an active member of ASHRM and recent nominee for their board of directors. I am a member of the United States Pharmacopiea ia Safe Medication Expert Use Committee - serving a 5-year commitment. And, lastly, I am a member of the Michigan Hospital Association Patient Safety and Quality committee.
Where do you see yourself in 5 years?
Doing what I have a passion to do - Healthcare Risk Management and Performance Improvement.

The latest Pearls has been developed this year by the pearls task force, chaired by David Benjamin MS, PhD. The task force, consisting of a team of dedicated ASHRM members, has applied a wealth of first-hand experience as practicing risk managers to create a solid educational resource for healthcare professionals on the prominent issue of medication error reduction.
"Pearls" for Medication Error Reduction has been designed to provide a quick and accurate reference on the most common medication errors, high risk drugs and problematic devices that can lead to medication error. The resource also looks at system errors, and explores how these errors lead to medication errors in different professions, including physicians, pharmacists, pharmacy technicians and nurses.
