Three Easy Steps to Becoming a Certified Professional in
Healthcare Risk Management
The Certified Professional in Healthcare Risk Management (CPHRM) Certification Program was launched on June 1, 2000,and since then 214 applicants have taken the examination. Interest in the American Hospital Association Certification Center (AHA-CC)/American Society for Healthcare Risk Management (ASHRM) program is ever increasing. An additional 47 applicants are registered to take the examination in the next three months. Fifty-seven risk management professionals sat for a ‘paper and pencil’ version of the examination during the 20th Anniversary ASHRM Annual Conference and Exhibition 2000 in New Orleans, and since then many more have taken the examination at 91 test centers around the country.
A WIDE RANGE OF SKILLS
The CPHRM examination was designed to provide risk managers with a tangible credential based on a solid assessment which verifies a broad-based knowledge of risk management. The CPHRM certification elevates professionals to elite status among peers in the industry, and many of the risk management professionals who have sat for the examination have described it as being very broad based.
The development of the certification program began some years ago when ASHRM member surveys expressed a necessity for a Risk Management credential. The membership at the time felt that a professional designation would help increase their visibility within their own organization, increase professional identity, and provide a tangible standard to measure competency in the field of healthcare risk management.

A THREE STEP PROCESS
“Eligibility requirements for participation in the CPHRM Program include a blend of education and experience” says

are provided whereby an individual can qualify to participate in the program. The pathways were developed to
be equivalent, providing fair and consistent treatment across candidates. To participate in the CPHRM examination, a candidate must meet the following conditions:
Work Experience - This criterion is met by one of the following:

A minimum of three years of full-time work experience in healthcare risk management in a healthcare setting or with a provider of services to the healthcare industry, with at least 50% of job duties dedicated to healthcare risk management

A minimum of 3,000 hours of work experience in healthcare risk management in a healthcare setting or with a provider of services to the healthcare industry, obtained over a period not to exceed five years
Education - This criterion is met by one of the following:

Distinguished Fellow of the American Society for Healthcare Risk Management (DFASHRM)
-
Fellow of the American Society for Healthcare Risk Management (FASHRM)
-
Registered nurse (RN)
-
Chartered Property Casualty Underwriter (CPCU)
-
Baccalaureate degree or higher
-
Associate degree, plus 25 points earned from among the categories listed in the Certified Professional in Healthcare Risk Management Candidate Handbook
-
High school diploma or its equivalent, plus 50 points earned from among the categories listed in the Candidate Handbook. This certification program in healthcare risk management includes the following content areas: loss prevention/reduction, claims management, risk financing, regulatory/accreditation compliance, risk management operations, and bioethics. Qualified risk management professionals interested in taking the examination should begin by obtaining a CPHRM Candidate Handbook. The handbook provides an outline of the process and includes the complete content outline for the examination, along with a list of review references that may assist in preparation, sample test items and the examination application.
The handbook can be downloaded from the ASHRM Web site at www.ashrm.org, under the recognition heading. Interested professionals can also call Applied Measurement Professionals at (913) 541-0400 to obtain a copy of the handbook.
Once a professional has reviewed the candidate handbook, the next steps are to fill out the application, schedule the examination and take the examination.
STEP 1: FILL OUT THE APPLICATION
The examination is offered on computer at several test sites throughout the United States. There are no application deadlines. Candidates who meet the eligibility requirements for the examination may submit their applications and fees at any time.
The application is included in the CPHRM Candidate Handbook. Required information on the application includes: personal information, indication of eligibility for examination, indication of the applicable fee, and candidate signature. Examination fees are $175 for ASHRM members or members of other Personal Membership Groups of the American Hospital Association, and $250 for nonmembers.
Once the application is completed, mail it and the appropriate fees to: AHA-CC Examination, Applied Measurement Professionals, Inc., 8310 Nieman Road, Lenexa, KS 66214-1579. Becoming an ASHRM member is an investment in your future. ASHRM membership costs $110 per year. Not only do ASHRM members receive a reduced fee for the CPHRM examination, but they also gain valuable benefits such as the quarterly Journal of Healthcare Risk Management, a weekly electronic newsletter, e-News, a bimonthly newsletter, The Forum, and access to the members only section of the ASHRM Web site, www.ashrm.org, and educational and networking opportunities unmatched in any other Risk Management professional society. STEP 2: SCHEDULE THE EXAMINATION
Once the application is processed by AMP, a confirmation of eligibility will be sent to the candidate within two weeks. The candidate's eligibility after acceptance of the application, is valid for 90 days. The confirmation notice contains a toll-free telephone number for the candidate to call to schedule an appointment for examination. The examinations are administered by appointment, Monday through Friday (except major holidays) at 9 a.m. and 1:30 p.m. at H&R Block offices throughout the United States. Specific address information will be provided when a candidate schedules an examination appointment.
The AHA-CC and AMP comply with the Americans with Disabilities Act and strive to ensure that no individual with a disability is deprived of the opportunity to take the examination. A convenient Request for Special Examination Accommodations form is available in the CPHRM Candidate Handbook for those professionals with special needs.
Once confirmation is received, the CPHRM candidate should begin preparing to take the examination. There are numerous references and tips available for this. The ASHRM Certification Education Task Force developed a Certified Professional in Healthcare Risk Management (CPHRM) Examination Study Guide, for use in preparation for the examination. ASHRM has also recently published the Risk Management Handbook for Healthcare Organizations, 3 rd Edition. Both of these publications are available through the AHA online store, at www.ahaonlinestore.com.
STEP 3: TAKE THE EXAMINATION
Once at the test site, a candidate will have identification verified and then receive a short tutorial on how to use the computer to answer test items. Computer literacy is not required. Only one test question appearing on the screen at a time. The candidate moves through the examination by clicking on the “next” key.
The candidate will have exactly two hours to complete the examination. When the candidate has completed the examination, the number of test items answered is reported. If a candidate has not answered all the items and has time remaining, they may return to the examination to answer those items.
The CPHRM examination measures a candidate's application of knowledge, not just the ability to recall facts. The AHA-CC and numerous ASHRM members with subject-matter expertise, worked diligently to create an examination that balances a candidate’s ability to recall, apply and analyze information over six major categories.
The examination is composed of 110 multiple-choice test items covering the above-mentioned categories. After the examination is completed, all candidates are asked to fill out a short evaluation of the testing experience. Upon leaving the test center, the candidate receives a copy of the score report indicating whether the candidate passed or failed the examination. In addition to overall score, subscores by major content category of the outline are reported.
BEST IN RISK MANAGEMENT
A candidate who passes the CPHRM examination is designated a “Certified Professional in Healthcare Risk Management” (CPHRM) and may use the designation in copy and in signature. Obtaining the CPHRM designation is an important step for many risk management professionals.
“I encourage any professional risk manager who meets the seating requirements to take this examination,” says ASHRM President Geri Amori, CPHRM, ARM, FASHRM. “Certification is one of the steps in the development of any profession in a unique field requiring special skills, special knowledge, and a standard for participation,” she notes.
CPHRM certification is valid for three years and renewable with the approved continuing professional education courses (45 contact hours during the three year period) or retaking and passing the examination.

PREPARING FOR THE CPHRM EXAMINATION

The AHA-CC recommends that review for the CPHRM examination focus on resources and programs that cover the information summarized in the examination content outline. Meet ASHRM Board Member Kathleen Knoppe
Risk Manager, Cook County Hospital, Chicago, Illinois
Organization: Cook County Hospital is a world-renowned academic medical center located in Chicago, Illinois. Cook County Hospital is celebrating 165 years in healthcare to the residents of the county. It was built at its present site at the turn of the century, that is, the last century. The hospital has upgraded services such as the emergency room as the standard of care evolved and added Trauma services when Trauma became of age. Cook County Hospital is the perfect educational training ground because of the vast cross-section of experiences and every day happenings at County. The County has been building a new facility to serve our citizenry with up-to-date technology in a facility that is conducive to learning. The new facility is located on-site behind the present facility and will be ready for occupancy in the summer of 2002.
How busy can this place be?
Cook County is one of four purely public hospitals in Cook County. Our figures were higher in 2000 than 1999. The ER saw 250,000 patients in the Adult ER and another 150,000 in the Peds ER. ‘Nough said?’ Not hardly. The Fantus Clinic saw over 500,000 patients in 1999. Now you know why I can say that I learn something every day. I have the opportunity to study in the world’s best classroom.
How does the real Cook County Hospital compare to ER’s County?
County Hospital sees adults in one ER and children in their own ER, which is staffed by Emergency Room Pediatricians and Pediatric Nurses with the assistance of other pediatric trained staff. Trauma is separate from the ER, unlike the organized chaos on ER where everyone comes blasting through the back doors. Education: I am an R.N., I have a Bachelors of Science degree in Health Arts. I have a Masters of Science Degree in Healthcare Administration and a Master of Jurisprudence in Healthcare Law. I am also an ARM, and I have earned my CPHRM.
Career Highlights: I was elected to my local chapter Executive Board in 1999 to serve a two year term. At the chapter level, I served on the Law Committee, the Publications Committee, and the Program Committee. On a national level, I have served on the Annual Conference Committee for both the San Diego and the Chicago conferences, and I have served on the Education Committee. I have the privilege of being elected to the ASHRM Board of Directors for 2001 - 2002. I have been blessed with meeting and communicating with awesome risk managers for almost eight years. My healthcare Risk Management experience encompasses a broad range of settings.
Personal: Master Gardener, University of Illinois Extension Service. I am a rose gardener, I have 28 rose bushes including English roses and antique roses! I am a test gardener for a large rose wholesaler. I also enjoy cooking for hungry folks. I live in a house built in 1877 after the Great Chicago Fire. My grandfather brought my grandmother here early in the 1920s. Since then four generations have begun life in this house and I am proud to be the present curator.
Career Advice: Keep your education up-to-date. I have each edition of the Risk Management Handbook, including the recently published third edition. I am amazed at how we have evolved. Without up-to-date education, the evolution could pass one by!

THE CHICAGOLAND HEALTHCARE RISK MANAGEMENT SOCIETY
The Chicagoland Healthcare Risk Management Society is entering its 20 th year. Plans are under way to pull together some historic information to share with the membership. Old members will enjoy remembering what has been done. New members will be impressed with how much has been done and how far risk management has come in the past twenty years. Our first education program for 2001 will be held the end of January and will address the HIPAA legislation. The Program Committee is already working on the annual meeting which will be held in April. Some type of social activity is being planned after the meeting to commemorate the 20th anniversary of CHRMS. Needless to say, we are hoping for a good turnout. The Nominating Committee met in January to begin pulling together a slate of candidates for this year's election.

ILLINOIS SOCIETY OF HEALTHCARE RISK MANAGEMENT
Illinois Society of Healthcare Risk Management’s (ISHRM) program, “JCAHO, HCFA, The Law & You!” held October 4th at St. Francis Hospital in Litchfield, Illinois was full of interesting topics and good participation. “JCAHO - Now and In the Future”, was presented by a JCAHO Surveyor. Issues surrounding the Patient Rights Standards, the Condition of Participation, and confidentiality were discussed. Participants of an actual HCFA validation survey shared their story so others could learn from this first hand experience. ISHRM’s spring seminar will be held in Peoria at Proctor Hospital March 1 and will center on regulation updates and loss control. The Annual Meeting will be held May 4 in Springfield, IL.

We are now 152 members strong! Many thanks to our membership committee, which has been ably led by Kathy Murray and will be lead by Denise Peterson in 2001. As President Elect, Bonnie Ellis not only chaired the Program Committee, but she also was our liaison to the New England Regional Risk Management conference and will be President when ASHRM 2001 comes to Boston. Mary LeBlanc, the Immediate Past President, quietly streamlined our nomination and election process (a true blessing), and we had many good souls who agreed to run for office. Karen Tourjee, Pamela Burger, Noreen McCarron, and Mary LeBlanc are finishing their terms on the Board and deserve our thanks not only for their specific projects, but also for their generous spirits and good sense. Finally, we had a great conference in Rhode Island from which many folks took home useful information.
PHILADELPHIA AREA SOCIETY FOR HEALTHCARE RISK MANAGEMENT
Sheila Stieritz is the chapter president for this calendar year, and Tom White is the recording secretary. On January 17 Pauline Rondeau (president of the Lehigh Valley Chapter and Director of Risk & Safety Services at Doylestown Hospital) was installed as Vice Chairperson of the Coordinating Council.
Meeting dates were established for the year as follows: 4/11, 7/11 (conference call) and 11/14. To add an agenda item, please channel it through your local Chapter or call Marie Conti directly at (215) 787-2423. In addition, November will be an open Board meeting followed by an educational presentation and lunch.
The Hospital & Healthsystem Association of Pennsylvania (HAP) Update included a presentation on HAP’s e-strategies with a request for suggestions as to what types of services would benefit members. Also, HAP's efforts on crafting Peer Review legislation was summarized. It will incorporate Penna, regulations on Reporting of Serious Events, as well as new JCAHO Standards on patient safety.
In addition, we pledged to enhance communication with ASHRM, the Hospital Association of Pennsylvania (HAP), and the Pennsylvania Association of Healthcare Risk Managers (PAHCRM).
SOUTHERN CALIFORNIA ASSOCIATION FOR HEALTHCARE RISK MANAGEMENT
Kelly Woodfin, an active and enthusiastic chapter member, was elected to the ASHRM nominating committee. Congratulations, Kelly. Following the October meeting, a joint session of the out-going and in-coming boards was held to facilitate smooth transition for the newly elected officers and board members. Chapter members were encouraged to join one or more committees, particularly new members, as a way of becoming an active part of the organization.
Chapter members have been treated to several excellent educational programs held in conjunction with the monthly membership meetings, including elder abuse in health care, introduction to alternative risk financing and managing the defense of sentinel events
The chapter’s annual educational conference will again be held at the Rancho Las Palmas in Rancho Mirage, from May 9 - 11, 2001. The theme will be “survivor”. Following the conclusion of the conference, a paper and pencil test for CPHRM will be offered; there will be an examination preparation track during the conference for those planning to take the examination.

Newfoundland chapter President Susan Sullivan is Regional Coordinator, Risk Management at Western Healthcare Corporation. She has been a risk manager for four years and has been a member of ASHRM for four years also.
When asked about the advantages to establishing an affiliated local chapter Susan notes that to be able to avail of the resources available with ASHRM is a great advantage. An affiliated chapter, notes Susan, also avails of the opportunity to network and learn from other chapters. To have the credibility and support behind us that being associated with ASHRM provides is important, says Susan.
What plans does the Newfoundland chapter have for the year ahead?
- To develop provincial proposals for certain risk management issues
- To continue to meet via teleconference every month, encourage educational opportunities and recruitment
- To seek funding and support in order to have an annual provincial meeting and educational opportunity.

Ohio Governor, Bob Taft (R), on January 9, signed into law House Bill 511, whistleblower protection legislation, which prevents disciplinary action against nurses who report unsafe patient conditions. House Bill 511 becomes effective April 8, 2001. Also in Ohio, House Bill 585, which becomes effective April 9, requires out-of-state physicians who regularly use electronic means to treat patients in Ohio to obtain a special telemedicine certificate form the Ohio State Medical Board. New telemedicine credentialing standards of the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) became effective January 1. NBC interviewed American Hospital Association (AHA) Senior Vice President Rick Wade on the pressures faced by hospital emergency rooms. In a Member Fax Update on the issue, Mr. Wade explains that many pressures, including labor shortages and hospitals operating in the red, are converging on hospitals at the same time. The update also notes that when an emergency department does need to divert patients to another facility, the hospital works with public safety officials to ensure that the sickest patients are sent to the hospital that is most able to treat the patient quickly. It also notes that a patient in urgent need goes to the closest hospital as a matter of policy. Risk managers and other healthcare professionals may obtain more information on this subject in the coming weeks on the AHA Web site, www.aha.org, under the section “Policy Forum.”
The Health Care Financing Administration (HCFA), on January 4, 2001, published the final rule addressing physician self-referrals. The rule, also known as Stark II, prohibits physicians from referring certain Medicare patients for 10 designated healthcare services to entities with which they have financial relationships, such as home health agencies and others. The rule, published in the January 4 Federal Register, takes effect January 4, 2002. The new rule is available at hcfa.hhs.gov/regs/physicianreferral/default.htm.
