PROGRAM
Modern healthcare often necessitates many caregivers for each patient. As a patient moves through the system, each hand-off/sign-over from caregiver to caregiver can be seen as a risk of lost information – or an opportunity to prevent mistakes down the line.
The April 10th program of ASHRM’s 2007 audioconference series will review some of the research on patient/staff transitions.
The faculty of experts will discuss models for communication and pitfalls to this process. Audience members will be able to take away some strategies to apply at their facilities … strategies that will allow them to monitor sustainability of the process and its applicability to the Joint Commission’s National Patient Safety Goals.

OBJECTIVES

* Convey the unexpected benefits of hand-offs/sign-overs as well as their hazards
* List at least three components of hand-offs/sign-overs
* Identify the responsibilities of the sending, transporting and receiving care providers
* Describe a minimum of four strategies to optimize information transfer
* Articulate two methods to monitor the effectiveness of the hand-off/sign-over communication.

* This program is approved for a total of 1.5 contact hours of Continuing Education Credit toward fulfillment of the requirements of ASHRM designations of FASHRM (Fellow) and DFASHRM (Distinguished Fellow), and toward CPHRM renewal.
* The American Society for Healthcare Risk Management is authorized to award 1.5 hours of pre-approved Category II (non-ACHE) continuing education credit for this program toward advancement, or recertification in the American College of Healthcare Executives.
