The Diagnosing Safety panel will review various tools and tactics for keeping your finger on the pulse of what drives risk at your organization. Panel members will review some of the commonly used techniques for collecting adverse event data, such as automated adverse event detection systems, voluntary incident report systems, surveys and hotlines. They will offer their opinions on the advantages and disadvantages of the different methods, and discuss options for organizing input to create reliable output. Don’t miss this chance to learn methods to uncover the critical information that your organization needs to help it get to zero and mitigate risk.

Objectives: At the conclusion of the session, participants will be able to:

  • Describe some of the various tools currently in practice to collect information about adverse events in healthcare settings
  • Explain how the tools are similar and different in terms of the methods for data collection, sources of information and the relative usefulness of the information
  • Translate the knowledge so it is useful for implementing new practices or improving existing practices in healthcare settings
