TEAM DISCOUNTS
ASHRM offers a 10 percent discount off the member or non-member rate for groups of 20 or more registrants from the same organization. All team registrations must be submitted together with payment to qualify for discount.
ASHRM MEMBERSHIP
Join ASHRM today and save $150 on Annual Conference registration. Membership in ASHRM is $140 per year and guarantees the best rate to the conference and other ASHRM offerings. Visit www.ashrm.org to join online. Please allow two weeks for processing. Once you receive your ASHRM member ID, you can register for the Annual Conference at the member rate. Please note that conference registrations and memberships are not processed together.
HOW TO REGISTER
(NOTE: Exhibitors must register starting in July using a separate form.)
Online:
Attendee registrations may be securely submitted online via https://register.rcsreg.com/regos-1.0/ashrm2004/adv/ga/. All online registrations require pre-payment by credit card.
Fax:
Fax a completed registration form to (805) 654-1676. All faxed registrations must be pre-paid with a credit card. Registrations that are not pre-paid will not be processed.

Registration Confirmation
A confirmation of conference registration will be e-mailed or faxed within 24 hours of receipt of registration. Please provide a fax number or e-mail address on each registration form to receive confirmation. Any questions related to conference confirmations should be directed to ASHRM at (312) 422-3980.
CANCELLATION POLICY
Registration fees, less a $75 cancellation fee, are refundable if written notice is postmarked no later than 10 business days before the meeting date – Oct. 4, 2004. No refunds will be made after Oct. 4. All cancellation and refund requests must be made in writing to: ASHRM, 1 North Franklin, Chicago, IL 60606; fax (312) 422-4580.
Contact the hotel directly to inquire about cancellation or refund of hotel deposits. ASHRM cannot be held liable for non-refundable airline tickets or hotel room deposits.
ASHRM reserves the right to reschedule or cancel a program because of an insufficient number of registrations or to close registration when programs are full. ASHRM may cancel a program when circumstances beyond ASHRM’s control – including, but not limited to, acts of God, governmental authority or war in the United States – make it impractical or impossible to host the program or may put participants in harm’s way. In such cases, ASHRM will refund one-half of the registration fee paid by the registrant.
Substitutions
Registrants unable to attend may send an alternate. If the alternate is not a member of ASHRM and the original registrant is a member, the non-member differential must be paid.
CPHRM EXAMINATION
Registration for the CPHRM exam is being handled by Applied Measurement Professionals, Inc. (AMP), the exam administrator. Criteria specified in the CPHRM Candidate Handbook apply to this offering. Visit www.aha.org/certification and click on the CPHRM logo to access the handbook.
Download an application PDF and mail your completed application with the required examination fee to: Applied Measurement Professionals, Inc. (AMP), 8310 Nieman Rd., Lenexa, KS 66214.
Applications must be received no later than Sept. 8, 2004. AMP will forward a confirmation letter no later than two weeks before the exam. If you do not receive a confirmation letter within two weeks of the exam, call AMP at (913) 541-0400. Late applications will be returned.
If paying by credit card, you may fax your completed application to (913) 541-0156. Please be sure to sign the application and check off the box that describes the correct examination type: “paper-and-pencil” and fill in the date of the exam: “Oct. 20, 2004.” Online registration for paper-and-pencil exams is not available. Examination fees are non-refundable. There will be no on-site registration.
Candidates confirmed for the exam should arrive on time and must present two forms of ID. One form must have a current photograph and the second form must have a signature. Candidates who arrive late or without proper ID may not be seated. Candidates may re-schedule the exam to computer-based training for no additional fee by calling AMP.
Scores for the paper-and-pencil exams will be mailed to each candidate within 6-8 weeks of taking the exam.
Any candidate with an ADA-recognized disability may request special accommodations by printing out Pages 19 and 20 of the Candidate Handbook and faxing them to AMP with the application.
